Vaccination Requirements

Immunization Requirements

By 6th grade, students should have the following vaccines:

• 4 doses of DTaP (diptheria, tetanus, and acellular pertussis) with 1 dose on or after the 4th birthday
• 4 doses of Polio with 4th dose on or after 4th birthday and at least 6 months after previous dose
• 2 doses of MMR (measles, mumps, and rubella)
• 3 doses of Hepatitis B
• 2 doses of Varicella (or evidence of immunity)

Vaccines REQUIRED FOR ATTENDANCE IN 7TH GRADE (usually given during the 11 year physical in 6th grade)

• 1 dose of TDaP (tetanus, diptheria, and acellular pertusis)
• 1 dose of MCV (meningococcal conjugate)

Vaccines REQUIRED FOR ATTENDANCE IN 12TH GRADE

• 2 doses of MCV (meningococcal conjugate
